These are the markers that determine whether atherosclerosis is being built into your arteries this decade.

ApoB mg/dL

Counts every atherogenic lipoprotein particle. The single best lipid predictor of future cardiovascular events — better than LDL-C in head-to-head studies.

Lp(a) nmol/L

A genetically determined, independent risk factor present in roughly one in five adults. Measured once in a lifetime, but often missed entirely.

hs-CRP mg/L

High-sensitivity C-reactive protein. The most useful systemic inflammation marker for cardiovascular risk stratification.

LDL-P / HDL-P nmol/L

Particle counts rather than cholesterol mass. Two people with identical LDL-C can have very different particle counts — and very different risk.

Type 2 diabetes does not appear overnight. It is the endpoint of a decade or more of insulin resistance — a condition that distorts almost every other metabolic process before it ever shows up on a fasting glucose. We surface it years earlier, when it is most reversible.

The single most under-ordered metabolic marker. Elevated fasting insulin precedes elevated glucose by years.

HOMA-IR index

A calculated index of insulin resistance from glucose and insulin. Quantifies metabolic dysfunction before it becomes diabetes.

A ninety-day average of blood glucose. The standard diagnostic marker for prediabetes and diabetes.

A snapshot of how your body handles glucose under steady state. Less sensitive than insulin, but standard.

C-peptide ng/mL

Indicates how much insulin your pancreas is producing. Adds context to insulin resistance vs. insulin deficiency questions.

Adipose-derived hormones that regulate appetite and metabolism. The ratio adds nuance to metabolic risk.

Strongly associated with metabolic syndrome, hypertension, and cardiovascular risk — not only gout.

A complete hormonal panel does not consist of TSH alone. Sex hormones, adrenal hormones, and thyroid hormones interact across daily and monthly cycles. Symptoms attributed to "stress" or "aging" frequently resolve when this panel is read properly and corrected.

Thyroid-stimulating hormone. The first-line screen, but insufficient on its own — the full thyroid picture requires more.

Free T3, Free T4 pg/mL · ng/dL

The active thyroid hormones. A normal TSH with low free T3 is a common and under-diagnosed pattern.

An inactive thyroid metabolite that rises under physiologic stress and blocks active thyroid hormone.

Non-alcoholic fatty liver disease is the most common chronic liver condition in the developed world, and the leading cause of liver transplant in adults under fifty. It is also entirely silent in its early stages, when reversibility is highest. The standard panel below catches it.

Alanine aminotransferase. The most sensitive blood marker for liver cell injury, including from fatty liver.

AST U/L

Aspartate aminotransferase. Read alongside ALT — the ratio carries diagnostic information.

Gamma-glutamyl transferase. Sensitive to oxidative stress and alcohol exposure.

Chronic kidney disease is the silent endpoint of hypertension, diabetes, and a few other common conditions. By the time it is symptomatic, treatment options narrow considerably. Standard creatinine alone misses early disease in muscular adults; we add cystatin C and protein excretion for resolution.

Forefront flagships are member-only centers operated to a different standard: private single-occupancy suites, in-house phlebotomy, and a three-business-day expedited turnaround. The first opened at the Diplomat Center, 820 Second Avenue in Manhattan; the second opens on Rodeo Drive, Beverly Hills, in 2026. Outside those cities, your panel is drawn at any of 2,000+ Labcorp patient service centers — same diagnostic infrastructure, same physician reading. The flagships exist for members who want the full Forefront experience in person.
Not strictly. Labcorp service centers accept walk-ins. We recommend booking an appointment for the convenience of choosing a time that doesn't sit in the morning peak (the busiest stretch is opening through 10 AM). When you confirm a Forefront panel, we send you a direct scheduling link for the location you've chosen. The Forefront flagships are appointment-only, with same-week availability for members.
Labcorp asks that you arrive about fifteen minutes ahead of your scheduled time. If you arrive earlier and a slot opens, they will usually take you sooner. If you arrive late within reason, the appointment generally still proceeds — though it may shift you later in the queue.
No penalty, no fee. Use the link in your confirmation email or text to modify the appointment, or message your Forefront care team and we'll handle it on your behalf. Forefront members do not pay reschedule fees regardless of how often plans change.
Labcorp closes individual centers when weather or operational conditions require it. You are never charged or penalized when this happens. Your Forefront care team is automatically notified of the disruption and will reschedule you at the next available time — or at a nearby center if one is open.

For most Forefront panels, yes — ten to twelve hours, water-only. Some specialty add-ons have different requirements. Your specific instructions arrive in your dashboard the day before your draw, with morning reminders. If you're unsure, message your care team rather than guessing.
Water is encouraged — hydration makes the draw easier. Plain black coffee is generally fine but check the specific panel instructions. Take prescription medications on your normal schedule unless your Forefront physician or your prescribing physician has told you otherwise.
A photo ID and your appointment confirmation. If you've chosen to apply insurance to qualifying specialty testing during your intake, bring your insurance card as well — otherwise it isn't needed. There's no requisition form to print: Forefront handles the order on the back end. You walk in, present ID, get drawn, walk out.
Yes. ADA-recognized service animals are welcome in every part of the patient service center.

From arrival to departure, most members are out within fifteen minutes. The blood draw itself takes two to four minutes. If you're a difficult stick, mention it on arrival — experienced phlebotomists are available at most centers.
For a standard Forefront panel, typically two to four vials totaling less than an ounce. Specialty add-ons may require one or two additional vials. The volume is well within safe limits even for members with smaller frames or who are sensitive to blood draws.
Yes. Spouses, family members, and care partners are welcome in the patient area at most centers. If you're prone to fainting or anxiety with needles, plan to have someone drive you and let the phlebotomist know.

Most Forefront panels are complete in three to five working days from the draw. Specialty add-ons sometimes take longer — up to two weeks for the most complex genetic and esoteric tests. You'll see results appear in your dashboard as they post, and your physician's written interpretation arrives in a single packet once everything is in.
Your dedicated Forefront physician. Unlike a typical lab experience where you are directed back to your ordering doctor — we are your ordering doctor. Every Signature and Private result includes a 1:1 review, by video or in writing. Essential members receive a written summary and can request a call.
Your Forefront physician is your first call. They will explain the finding, recommend next steps, and — if appropriate — coordinate with your existing primary care or specialist team. Forefront is preventive medicine; we are not a substitute for emergency care or for treatment of acute disease. For anything urgent, we direct to the appropriate setting.

Privacy Policy

What we collect

Forefront collects information you provide directly (membership application, intake forms, communications with your care team), information generated through your use of our services (lab results, dashboard activity, AI conversation history), and limited technical information from your device (browser type, IP address, page interactions).

Lab results processed through Labcorp are subject to Labcorp's own privacy practices in addition to ours. Your Labcorp data flows to us only with your authorization.

How we use it

We use your information to deliver care: to interpret your lab results, draft your protocol, surface clinically relevant patterns, respond to your messages, and improve the operation of our services. Aggregated and de-identified data may be used to improve clinical tools and AI models.

What we don't do

HIPAA & security

Forefront is a HIPAA-covered entity. Your protected health information (PHI) is encrypted in transit (TLS 1.3) and at rest (AES-256). Access is restricted to your clinical care team and authorized operational staff bound by HIPAA. We maintain Business Associate Agreements with all subprocessors who touch PHI, including Labcorp.

Your rights

You may request access to your data, correction of inaccuracies, deletion of your account, or a complete export of your records at any time. We respond to such requests within 30 days. Members in California, Colorado, Virginia, and other states with applicable privacy laws have additional rights under those statutes, including the right to opt out of certain processing activities.

Terms of Service

The agreement

These terms govern your relationship with Forefront Health, Inc. ("Forefront," "we," "us") when you apply for membership or use our services. By submitting a membership application or using the dashboard, you agree to these terms.

What Forefront is — and isn't

Forefront is a preventive medical practice that provides diagnostic testing, longitudinal biomarker tracking, written protocols, and access to clinicians and coaches. We are not a substitute for emergency care, primary care, or treatment of acute disease. For medical emergencies, call 911 or proceed to the nearest emergency department.

Lab specimens are drawn and processed through Labcorp's network of CLIA-certified patient service centers. Forefront orders, interprets, and acts upon the results; Labcorp performs the laboratory analysis.

Membership & payment

Forefront membership is a flat annual fee, paid in advance or in installments. Membership is cash-pay and HSA / FSA eligible. Insurance may be applied to qualifying specialty testing during your intake; insurance is never required for membership itself.

You may cancel your membership at any time. Pro-rated refunds are issued for the unused portion of your annual term, less the cost of services already rendered (panels drawn, protocols delivered).

The clinician relationship

By becoming a member, you establish a clinical relationship with a Forefront physician licensed in your state of residence. Your physician orders your panels, signs your protocols, and is available for consultation. Forefront does not provide care in states where we are not licensed; if you relocate, we will work to transition you to a licensed provider in your new state.

Specialty testing & referrals

Some specialty assays from Labcorp's catalog are recommended only with specific clinical indications. Your Forefront physician will discuss whether a given test is appropriate before ordering it. If your panel surfaces a finding outside the scope of preventive medicine, we may refer you to a specialist or your existing primary care provider. Referrals are billed through the specialist's practice in the usual way.

AI features

Forefront uses AI to surface patterns in your data, draft protocol recommendations, and respond to routine member messages. All clinically material AI output is reviewed by a Forefront physician before it reaches you. You may opt out of AI features at any time; opting out may extend response times and reduce protocol detail.

Limitations of liability

Forefront's liability for any claim arising from your membership is limited to the fees you have paid in the prior twelve months. We make no warranties about specific health outcomes. Preventive medicine reduces but does not eliminate the risk of disease.

Disputes

Any disputes arising from your Forefront membership will be resolved through binding arbitration in the State of New York, except as required by applicable consumer protection law. You retain your right to bring claims in small claims court.

Forefront is building a small, distributed practice for board-certified physicians who want to do preventive and longevity medicine well — with the time, the data, and the structural support to actually do it.

Most preventive medicine in the United States today is rushed, undercompensated, and structurally subordinate to disease-driven workflows. A clinician who wants to read advanced biomarker panels, author detailed protocols, and follow members across cycles is fighting their EMR, their schedule, and the reimbursement model to do so.

Forefront is the practice that removes those fights. Members pay an annual membership directly. Our physicians see fewer patients, with longer relationships, supported by clinical AI for pattern detection and a care team that handles intake, scheduling, and follow-up. The work is preventive medicine, done at the standard the literature actually supports.

Panel size

A capped book of members.

Each Forefront physician carries a deliberately small panel — sized for the depth of practice the membership promises. No volume bonuses, no productivity quotas, no incentive to compress the work.

Compensation

Base salary, not RVU.

Forefront physicians are salaried, with performance reviewed against member outcomes and care quality — not visit volume. Equity participation for early hires. Full benefits, including malpractice and CME.

Workflow

AI on the chart, physician at the wheel.

Our clinical intelligence layer surfaces patterns, drafts protocol revisions, and pre-summarizes member data for review. You sign every clinical decision. The AI handles the legwork, not the judgment.

Time

Real visits, in real time.

Intake visits scheduled at 60 minutes. Follow-ups at 30. Documentation drafted by clinical AI for your review, not authored from scratch by you. The administrative load that pushes physicians out of medicine is structurally reduced.

Practice setting

Distributed, licensed widely.

Forefront is a telemedicine-first practice. Physicians work from a state of their choosing and hold licensure in the states their members reside. Lab work is drawn at any of 2,000+ Labcorp service centers.

Who we look for

Board-certified, genuinely curious.

Internal medicine, preventive medicine, cardiology, endocrinology, or lifestyle medicine. Comfort with longitudinal data. A view that the standard of preventive care should be much higher than it is.
